Treatments
1. Vaginal moisturisers

2. Systemic HRT
For symptoms which are related to hormonal changes, hormone replacement therapy can really help. Systemic HRT which consists of a topical (skin) patch or gel or an oral tablet can replace low oestrogen. If you still have a womb, then you will also need a progesterone to protect the lining of the womb, usually in the form of a tablet or a hormonal coil.
3. Topical (vulvovaginal) HRT
Topical oestrogen delivered locally to the vagina and vulva is helpful in reducing symptoms of itching, burning, pain during sex and urinary symptoms. Topical oestrogen is incredibly safe as the dose is so low and so little is absorbed into the rest of the body. Sometimes local oestrogen is needed in combination with a systemic (tablet or skin based) HRT.
4. TRT for women
Current guidance recommends starting with hormone replacement therapy (HRT) using oestrogen, with progesterone if needed, before considering testosterone.
Not everyone absorbs hormones through the skin effectively, so a blood test to check your oestrogen levels on HRT and your natural testosterone level is key. For many women, adequate oestrogen stops vulval and vaginal irritation, making sex more comfortable and often restoring sexual pleasure and desire naturally.
If you’re still struggling with low desire despite proper oestrogen levels, and other factors like psychological, social, or relationship issues have been ruled out, a trial of testosterone therapy could help. Typically, this involves a small daily dose of testosterone gel, about an eighth of what men use. It’s low-risk when monitored by an experienced doctor, with blood tests after three weeks to ensure testosterone levels stay in a healthy range.
